Mt. Sinai COVID PCR Testing Program
All Eastchester Staff and Students are eligible to participate in this free COVID testing program. Each individual who enrolls will be provided with a self-administered saliva PCR test kit every 1-4 weeks. Once completed, the test kit is deposited into a drop box located within each school. Results are emailed to individuals within in 24-48 hours. The more people who participate in the program, the more able we are to detect asymptomatic COVID cases and reduce transmission.
Enrollment is simple. First, please read this FAQ about the program, then:
Families should complete this Consent form for their children to participate.
Families should complete this Opt Out form for their children if they no longer wish to receive weekly test kits.
Staff and Faculty should complete this Consent form to participate.

Once your form is processed, you will soon begin receiving a labeled test kit every 1-4 weeks.
FOR ENROLLED STUDENTS: Test kits should be taken home each Monday. The test should be taken Tuesday morning and the kit returned to the deposit box at school before 10 a.m.
FOR ENROLLED STAFF: Tests should be taken at school on Monday or Tuesday and deposited into the deposit box before 10 a.m.
