If you are a staff member and have tested positive for COVID, please complete this form. After you have completed the form, you will be sent an email with personalized information including return to work dates and information on attendance reporting.
If you enter a submission after work hours, you can expect a response by 9PM on workdays. If you enter a submission on a weekend or holiday, you will receive a response the evening before the next workday.
