OMNI and 457 plans

Employees have the ability to contribute to a 403(b) and/or 457 plan of their choice via payroll deductions. All new hires must complete a form even if they are not interested in participating.

If you are interested in contributing to a new or existing 403(b) plan, please download, print, and complete Parts 1, 2, and 5 of this form and return to the Human Resources Department:

If you are interested in contributing to a new or existing 457 plan, please download, print, and complete this form and return to the Human Resources Department.

If you are planning on retiring, you will need to contact the appropriate retirement system for information on your pension and eligibility.

New York State Teachers' Retirement System (NYSTRS) is the retirement system for certificated staff. Membership is required for all full-time certificated staff, and optional for substitute employees and part-time staff members.
